If your Event requires more than the Standard Risk Assesment (see your Visiting Company Presenter’s Agreement), you’ll need to populate this template (or a similar one) with your own detailed, accurate Risk Assessment. Open this folder and right-click the file (ctrl + click on a Mac) to Download it as a .docx. You can view an Example Risk Assessment (below) to get an idea of how it should look when it’s finished. Make sure to be completely honest and comprehensive in your assessment, and make sure to get it in by the required technical deadline. Failure to submit a valid risk assessment will mean your performances will not go ahead.

If your Event uses copyrighted music, you are required to complete a PPL / PRS Declaration (with the exception of Lates events, for whom we factor PRS costs into the Deal). Please see the Production & Technical Guidance document above for a full explanation of which of the two forms linked here you need to complete. To download the templates, open the folder linked here and right-click the file (ctrl + click on a Mac) to Download it as a .docx.
